Please fill in all information that applies to you. Exhibit registration must be received by October 15, 2018 and must include this completed form and full payment. If you plan to participate in other activities of the WCB convention, you must also complete a Convention Registration form on the WCB web site.

If tables are overbooked, your exhibit may be limited to one table.
Check the box if you wish to have access to power; extension cords are not provided by WCB or the Crown Plaza hotel.

Important Information: Exhibitor Agreement

Vendors are expected to participate in, and are solely responsible to man their table for, the entire event – friday, November 2, 2018, 10:00 am-5:00 pm (closed during the noon hour for lunch). In no respect shall wcb be held liable for any damages, perceived or actual, incurred by any vendor as a result of their participation in the wcb convention. Vendors representing blindness related products and services receive priority. Vendor cancellations must be requested in writing no later than October 19. Email: wcb.exhibits@gmail.com with “cancellation” in the subject line. A $10 processing fee will be deducted from cancellation refunds.
